Name Change of a Minor

My daughter has her father's last name and my son has mine. I'd like to change my daughter's last name to mine and I have full custody. Do I need the father's consent?

Yes in Arizona you need the permission of the other parent in order to obtain a name change. Additionally, your decree may specify what name the child will be referred to. Unless there are exigent circumstances that would require your child to change his name, you would be required to obtain the Father's permission.
