Legal Question in Family Law in Arizona

Name changes

How do I go about changing my last name back to my maiden name? I have kept my married name after a divorce, and would now like my maiden name to be my legal name.

Re: Name changes

You have the option of asking for a modification of your Decree, reinstating your maiden name, which would be a simple process if your decree is recent and if your ex-Husband would "stipulate".

Otherwise, you could file a Petition for Change of Name (which is filed in the "Probate" Court).

Re: Name changes

You have a couple of options. Depending on how long ago your divorce was final, you may be able to ask the court to modify your divorce decree to reinstate your maiden name.

Otherwise, you will have to file a separate Petition for Change of Name (this is a civil filing as opposed to a family law filing) and follow the court's procedure for giving notice, having a hearing, and ultimately having the judge change your name.
