Changing Primary Custody

My ex and I have joint custody of our daughter, I want her to come to live with me due to the issues she is having with her father. How do I go about getting this done? She is 11 and cannot get along with her father, they scream and holler. I am afraid this will escualte to worse if I do not step in.
