Legal Question in Family Law in Arizona

Is there a consent form that I can have the father of my child (9years old) filled out so that my husband who has been raising my son with me for the past 6 years can adopt my son? My son was born in Arizona and we currently live in Germany. The childs father does not have any contact with my son and does not pay child support!
