Contempt of Preliminary Injunction
I filed for divorce and my husband
was served papers. Since being
served, he has cancelled our joint
checking, reported that I am using
my debit card fraudulently and has
cancelled me from his medical
coverage at work. How do I file for
contempt?

It is unclear from your question whether this is a WA case or an AZ case. The language "Preliminary Injunction" suggests it is an AZ case as that is not generally used language in WA in family law cases.
