If a father is refusing to give their child the original birth certificate or insurance card. What do they do?

It is very easy to get a duplicate birth certificate from the department of vital records.
Regarding insurance, if he refuses, perhaps you could call the provider and see if they will send duplicates. OTHERWISE, I would suggest a demand letter from an attorney. ULTIMATELY, you could file in Court a demand that he provide the card, but I would of course hope you not need to go that far.
