what happens if you forgot to include an annuity in a divorce?

That can be corrected. Under Arizona law, any community, joint tenancy and other property held in common for which no provision is made in the Decree "shall be from the date of the decree held by the parties as tenants in common, each possessed of an undivided one-half interest.".
Perhaps an agreement can be reached wth your ex-spouse as to how to address this asset? If not, a Petition or Motion can be filed in Court to have the matter addressed.
