What forms do I use to file for a mutual consent divorce of a Covenant Marriage?

If you are using the self service center documents, you look for "Consent Decree".

The forms and instructions for filing are located at www.superiorcourt.maricopa.gov, self-service center. There are separate forms for "covenant marriage" and "non-covenant marriage."
