Legal Question in Family Law in Arizona

Good AM - Found out from a nice lady attorney in Chandler that my son needs to file a petition to prevent relocation so his ex-wife can't take their 2 kids and move to Colorado - they have joint custody. We have found out that there is no form - we must create one. What should be included/not included in this petition and what formate should it be in.
