is it legal in arizona for a grandparent to leave state permenatly w/o concent from parent. or without the mothers knowledge?

No. Unless she has some sort of authority, that may be a crime. Perhaps a phone call from us, or a demand letter explaining the wrongful nature of her conduct, would be enough.
