I live in AZ and was divorced 3 years ago. I want to change my name back to my maiden name but it isn't in the divorce decree. Please help!

Because of the amount of time that has passed since your divorce, you may have to file a separate application to change your name in the civil court. The process is usually straightforward. You can follow this link to get to the forms/instructions:
