Legal Question in Family Law in Arizona

I live in Maricopa County in Arizona, and I plan on paying off my child support arrears. I am a little confused on how I am actually supposed to go about it. I figured out how much arrears I owe (interest included), and I have filled out an "agreement to modify income withholding order". My ex and I are both going to sign it in front of a notary, and it is a mutual agreement that the amount I am paying is correct. There is no problem there. My main concern is that 7b on the "agreement to modify income withholding order" asks me to list and attach any supporting documents. My question is if it is actually necessary for me to provide any supporting documents if this is a joint agreement and there is no disagreement between my ex and I. Do I still need to provide a certified payment history? Do I need to include an arrear calculation sheet from the clearning house I mail payments to?

You are not required to attach supporting documents if there are none. If both of you are in agreement, the notarized document should be sufficient.
