minor name change
My daughter has my maiden name, I would like to change her name to my married name. We have two other children and everyone has the same name except her. Her biological father is not on the birth certificate. He would most likely consent to the change. How do I petition the court? Where else do I get the forms besides the internet?

Sometimes you can get a clerk of court to do a name change case search for you and let you look at the file. If not, watch the papers for a name change notice, then ask to look at that file. Law libraries may also have forms, as do office supply stores and Nolo Press. Be aware you will need to follow the statutes, so you will need to read those. And if you live in WY you will probably have to change your daughter's name in Wyoming.
If paternity has not been established, and the father's name is not on the birth certificate, you should be free to use the last name you choose for your daughter, without a formal name change.
