Legal Question in Family Law in Arizona

What are the proper legal forms to file in an attempt to ask the court to intervene in decision making regarding choosing a school for a minor child if the parents dont agree which have joint custody and equal rights in making the decision; and if there is a stipulation in their agreement which states the parties are to seek mediation in the event of disagreement and one party refuses and takes it upon themself to enroll the child in the school of their choice and refuses to adhere to, comply with, the mutually agreed upon conditions of the order. The other party withdrew the child from the school I placed him in after withdrawing him from the school the other party enrolled him and and for two weeks the child has not been in any school and I am hesitant to enroll him anywhere because I don't want the child to continue suffering any form of injury by all the nonsense going on.

It is difficult to point you to a specific form. If you are using self service center documents, perhaps a "Petition to Enforce".

If I were filing for you, I would file an Expedited Petition for Order to Show Cause, which would explain the issues and request an expedited hearing.
