In AZ, you can ask for termination of parental rights on the grounds of abandonment if there has been failure to maintain contact for a period of 6 months prior to the filing of the petition. Failure to support (whether or not there is a court order) also contributes to a finding of abandonment.

You clearly have a basis for "Termination" of parental rights, which means that Father would thereafter have NO rights towards the child. The steps are rather simple.
