temporary guardianship and temporary POA for any health matters

is there a form that covers both : temporary guardianship for the care and medical needs of the child.

If you expect someone to just care for your child temporarily while you are out of town, just write out your wishes and expectations and sign the document which the care provider can keep in their possession. You might wish to provide a copy of the child�s medical insurance cars, also. I can prepare such documents for you, but this would involve a fee.

If this is a temporary, but lengthier placement of a child in someone else�s care, you should consider getting a court ordered temporary guardianship. You can hire a local attorney; I practice in Southern Arizona so I would not be able to go to court with you. Or you can take court action on your own. If you qualify, there may be a local Legal Aid clinic you could attend where they explain how to fill out the paperwork and go to court on your own. Check at azlawhelp.org for your local Legal Aid office.

If those two options don�t work, you can file the action on your own in Court. The Maricopa County Self Help Center has sample forms and directions. See http://supreme.state.az.us/selfserv/. If you don�t live in Maricopa County, replace Maricopa with the name of your county. There will be a filing fee at the Clerk of Court and then they will set a hearing date for you to see the judge.
