termination of fathers rights

How can i terminate the rights of a sperm donor father His name is not on the birth certificate, he does not pay child support, he did not go for his blood tests when he was served for child support, he has only seen my child twice in almost 6 years. Thank U

Very simply, you can file a "Petition for Termination of Parental Rights". Suxh is filed in the Maricopa County Juvenile Court. You clearly have the basis for such. The only potential issue is to confirm that you are terminating his rights so that the child can be freed for POTENTIAL adoption by a fianc� or Husband.
