Termination parent-child relationship

I have guardianship of two grandchildren and sole custody of their older sister. Circumstances are such that I need to immediately file for Severance / Termination of parent-child relationship for them.

I am having major difficulty locating the forms to file this with. Do you have any solutions to help me locate these? I cannot afford a lawyer to help with this and every agency that offers assistance does not handle this type of case or assist with them. I am desperate and need immediate advice on how to obtain these forms.
