Voluntary Termination of Parental Rights

I am trying to terminate my right with a former girl friend that i had a child with we filled out a form for adoption consent but I dont think she filled it like she said no she is married and is in agreeance to have my rights terminated but she is not be reponsable with getting the forms to do so and it has created a judgment on my credit for child support and they are taking the money out of my tax return I am trying to see if I can get the form and fill it out and send it to her to get this done so I can fix my credit to purchase a house and this is what is stoping me please help

If you can get the right forms, fill them out correctly and get the judge to sign them, that's good. Otherwise it results in a judgment on your credit and garnishment of your tax return. Seems to me like it would be more effective to hire a lawyer who knows how to do this and get the job done once and for all. But you can do whatever you think is more economical.
