In an accident, does the fault always go to the person who rear-ended someone, or are there exceptions?

Generally causing an accident by rear-ending another vehicle is negligence per se, meaning it is negligent because it violates a law or statute. it is possible there could be other factors involved, such as another vehicle hitting the vehicle and then causing that vehicle to rear end a vehicle. It is really dependent upon the facts of the incident, but generally if you are driving your vehicle and you rear-end another vehicle, you are at fault.
