do i have to change my will if my address and daughter's name has changed?

No, you don't. There may be other good reasons to make changes in your Will, ie. your daughter's circumstances have changed (not just her name). You should have a consultation with an experienced lawyer who can help you determine what your needs are today, and if you need other documentation or legal authority so that you can take care of your spouse, or your daughter, or that they can take care of you in a case of accident or illness. More important than having a Will, is having a durable Health Care Power of Attorney and someone designated to take care of you, should the need arise.
