Where would I obtain a legal form for obtaining a deceased persons personal belongings from a care home?

I know of no such form. If there is less than $50,000 worth of assets in the estate of the deceased, then you can file an affidavit with the court which would allow you to collect the personal property of the deceased. There are forms and instructions at the Self Service Center of the Maricopa County Superior Court website, and if you are outside of Maricopa, you could probably use the same forms.
