How to fill out an affidavit

I need to fil out a few affidavit,never done this before. They will be submitted to holders of accounts I.E. mutual fund,money market and a bank. these account wre owned by my deceased father. Ineed to have them, account holders,alow me to re-title the accounts into his trust.

you have to have legal authority to act on behalf of a deceased person. You should file a petition for appointment as personal representative of the estate, and then you can sign affidavits and transfer assets or accounts into the trust.
