Form to request copy of trust

I have been named a beneficiary of a Living Trust. I need to request a copy of the Trust documents.

Is there a form I may use / copy? Do I need to have it notarized to PROVE that it is ME requesting it?

Your question does not state it, but I presume that the trustor (creator) of the trust is deceased, therefore you are now entitled to benefits and a copy of the trust.

There is no "form" needed. The trustee should provide a copy of the trust to all beneficiaries. You can send a letter requesting same. Also request a copy of the will (there is usually also a will) and if someone has acted as agent under a power of attorney during the life of the trustor, and acted for his or her benefit, you can request a copy of the accounting for that service as well.
