Irrevocable Power of Attorney

I have been searching for a sample form of a ''irrevocable power of attorney'' and have had no luck.

Does such a thing exist and if so could you please email one to me.

There is no such thing as an irrevocable power of attorney. A POA is an agency agreement. The principal can always revoke the authority of the agent, and if the principal is incapacitated, the POA automatically is terminated unless by its provisions is deemed to remain in force. I advise you to be very cautious about using any form without really knowing the law that applies, and you must know how to validly execute a power of attorney. Also, be aware of the criminal and civil liability which may befall the agent acting under a POA.
