what type of trust or other document would protect a
small estate from catastrophic health problems?
where medicare would not cover it, such as nursing homes for alzheimers, which would leave the survivor
destitute?

The protection you can achieve depends in large part on the circumstances. Generally, there are 2 possibilities -- insurance and asset protection planning. How effective the planning can be depends on several factors. Generally, it is difficult to do much planning after the catastrophic condition has occurred. You need to be careful here, because there are criminal penalties for transferring assets in order to obtain greater benefits.

There are specific insurance policies that would pay the cost for the type of care you mentioned thus sparing your estate. I suggest you contact a life and health insurance agent and ask him or her your questions. I know some excellent agents that would be more than happy to discuss your needs at no charge. Regardless of the size of your estate, you will want to make sure to protect it for the benefit of the surviving spouse, then for your heirs.
Nursing home insurance is one option. There are other options with respect to meeting entitlement guidelines for Medicaid that might also be appropriate in your situation.
