Legal Question in Wills and Trusts in Arizona

I have a Will already in place which was done by an attorney. I need to change the

name of the person in charge of my medical decisions since that person has left the state.

Is there a simple way of changing the name and having this to be an addendum of some sort?

Can I do this with your form and and have it notorized by two peple?

A Will does not give anyone authority to medical decisions for you. If you have a durable health care/medical power of attorney, to change the name of the agent named, will require that you make a new POA, which is witnessed by 2 persons in front of a notary public. I do not give out any forms to clients, that is part of the services I render.
