Legal Question in Wills and Trusts in Arizona

My 34 year old daughter is a single mom and has nothing in writing to convey her wishes about what should be done if she suddenly dies. Is there a form she could use to name a personal representative for her estate and to say who she wants to raise her two teens?

Last Will and Testament.
Be careful. Many people who use a "form" instead of an attorney do not get the proper wording, signing and content to make it work.
