Re: who do I contact

Not as simple a question as you might think. First, your doctor has to take you off work for more than seven (calendar) days to get any compensation at all. Mere reduction in earnings, while continuing to work, is sometimes hard to get compensated.

Second, the insurance carrier is responsible for paying you, if you are entitled. You would call them and speak to the adjuster on your claim.

Third, if the carrier does not respond satisfactorily, you would contact the Industrial Commission of Arizona (602)542-4661. That is the state agency that regulates workers' compensation and has the authority to tell your insurance company to pay.

Back to point one, however, the most important thing to do is clarify your work status with your physician.
