Leave of Absent form

I have a work related injury which I have had my first surgery and may face more. My employer a large building materials company has requested me to sign a leave of absent form. Is this a normal form that a person out on workman's comp would be expected to sign?

Your question raises a point that many injured workers are surprised to hear. In Arizona, employment law is largely separate from workers' compensation law. The question of whether your employer can ask that you fill out that form is, primarily, an employment law issue. Under workers' comp, your signing of that form will have no affect on your benefits.
