What if after all the physical theraphy and other treatments fail and the doctor says your only hope is surgery or live with the pain. What options do I have?

You do not provide me with enough information to really answer this question. I take it that you don't want surgery. A few things you should know are...
1. Just because your doctor says you need surgery doesn't mean that if you decline it you will be compensated for disability.
2. Your insurance carrier would probably send you to another doctor for an opinion, even if you elect surgery.
3. If you do end up eligible for permanent disability compensation (either total or partial disability), regardless of surgery, what you get can vary widely, depending on the body part and many other factors.
