Legal Question in Criminal Law in Arkansas

privacy

The company I work for just gave us a form to sign that concerns me on privacy. It states when you go for a doctors appointment you must bring back a note signed by the doctor and a statement as to the nature of the visit, migrane,bladder infection etc. I have no problem with the excuse just the privacy of my doctors visit. I do not feel it is the company's business as to the nature of my doctors visit as that is private between you & your doctor. The form states if you do not provide the company with this information you will be terminated. I believe this is going against mine & my doctors right to privacy.Is this legal for them to require me & my doctor to provide them with that information?

NO! The only thing they can require is something from the Dr stating that you had an appointment with him at such and such a time and perhaps that you left there at such and such a time. Your employer is not entitled under HIPPA to know what your purpose with the dr was, or any other medical information unless you grant it voluntarially. Under threat of termination is not voluntary!
