Need Adoption Form For Current Class
Need to create a form for a Tech Writing class allowing
Private Party Adoptee Birth Mothers to make available to the child all or part of their contact information and also any relevant information about the Father, why they were left for adoption, ''things you should know'',... period
This information to be given directly to the child upon reaching the age of majority or provided to the adoptive parents upon request.

there is already a form the state of Ca uses called AD 67 and AD 67a which records the social and medical history of the natural parents and the child has access to this information through his adopted parents. The state will also make the information available if both the natural parent and the child register with the state. look at the form and it will at least give you an idea what information to include in your form DSS has a web site to get the from in adopbe pdf format.
