if a court document says one parent is the primary caregiver and the other has liberal visitation, what does that mean?

I'm curious as to what type of "court document" this is. Is it a divorce decree, temporary order, final custody and visitation order...?

Unfortunately there is no quick and easy answer to your question. That is why I always urge my clients with custody and visitation issues to include a very specific visitation schedule in the divorce agreement or proposed order. Even with clients that insist they can work out visitation with the other parents, I urge them to include a detailed schedule to use as a default if they cannot agree on visitation later on. If it is spelled out in detail in the order, then there is no question about who has the child(ren) when, and therefore no need to go back to court and sort it out. Family circumstances are constantly changing. Children have different needs as they age, and often one or both parents remarry or become involved with someone, and this causes conflict.
