Forma Pauperis

I can not locate forms that allows one to go into court under the above. I found the Motion to proceed on appeal in forma pauperis which is an affidavit! Is this also the main motion for a Forma Pauperis. I can not find a form that shows me how to file a proper Tort Action.

I don't know if there is a form for a proper tort action. You might consider going to the courthouse to ask the clerk to let you copy a tort that an attorney has filed. Or, if you are near a law library, they may have some examples. Or, if you know an attorney, he or she might help by letting you see a tort action.
