Legal Question in Appeals and Writs in California

I was denied a motion to set aside or vacate a default judgment in a limited civil case. I want to appeal that order. I have found the form APP-004 to file my appeal, but I do not know if I am filling out the form correctly. I do not want to make a mistake. Is there any way you could inform me on how to fill out the appeal form? Your help would be greatly appreciated.

No. Legal advice is not to be given in exactly these type of matters becasue a mistake can be made. You must seek and interview attorneys willing to take your case and then let them file your appeal.

If you act as your own attorney, the law does not protect you for making mistakes because you do not know how to fill out a form correctly.
