there are no forms. you write and file a notice of appeal and then you write an appeallate brief.

If this is a small claims appeal, ask the clerk for the form. If a superior court appeal, there is no "form" as such; see Calif. Rules of Court, Rule 1. You must specificy what final order/judgment you are appealing from and make sure you file the notice of appeal timely. There are strict time limits to do this.
