I am really sorry, but your post is virtually uninteligible. I assume you are asking where can you find the forms on the internet? If so, you can go to the bankruptcy court closest to you (for zip code you provided, that is the Riverside branch of the Central District of California), or their website which is www.cacb.uscourts.gov and download forms there. Please understand that bankruptcy is an extremely complex legal proceeding, and going into this without fully understanding what you are doing is foolish. Failure to comply with the rules can actually end up being a Federal Crime, so please consult with an attorney rather than trying to do this yourself. Most attorneys are not that expensive when compared to the relief you can obtain through a properly prepared and filed bankruptcy.
