is there & what is the form you need to file with your bankruptcy for creditors that have a judgement?

I think what you're thinking of is a document that you would file with the court that issued the judgment, and serve upon the attorney(s) for the party that obtained the judgment. It's called a "Notice of Stay of Proceedings" and can be found at: http://www.courtinfo.ca.gov/forms/fillable/cm180.pdf . Make sure to list the creditor on bankruptcy Schedule F, along with the attorney's address.
