Legal Question in Bankruptcy in California

I am just about done with all my forms for chapter 7 bankruptcy filing. Since I am un-employed and I have received no wages from any employer in the last 60 days, I have checked the second check-box in Statement of Payment Advices form for my district (Northern California, San Jose).

I have been receiving unemployment for the last 4 months and I would like to know if I should attach proof of that income with my initial filing with the clerk, or attach a copy with the payment advices form.

Any help is greatly appreciated.

Yes. There may be a form for unemployment benefits reporting (there is one in the Central District).

This raises an important point for the readers: bankruptcy may be governed by federal law, but it is also a function of state law and very, very detailed local practices (specific to bankruptcy districts). This means that what flies in my birthtown of San Jose won't necessarily go in my San Diego stomping ground.
