Who To List As A Creditor For Individual Bankruptcy

I am filing bankruptcy as an individual and doing it through a software program. I am confused as to who a credit actually is in certain instances. One example is a collection action. It is from an Apt. management company, but shows up on credit reports by the law firm that initiated the court action. Also, there is now a county sheriff in charge of collection. Who do I list as creditor? Also, when a company refers an account to collection, who is the creditor? The original company issuing credit or the collection agency?
