Commission for Referals
I would like to find a form agreement that would satisfy the following fact pattern:
A company needs an individual to make introductions to key people in a certain industry. In return for those introductions, the company is willing to pay this individual a 10% commission on any sale that results from his introductions.
Does anyone have such a form? Does anyone have a template that I might use as a sample?

No. I would be astonished if any attorney would give away his intellectual property that way. Further, such contracts need to be customized. You can give me a call.

A fairly standardized independent services agreement should give you a "skeletal" template of how it should appear. However, these type forms are generic and usually do not always encompass EVERYTHING that needs to be agreed upon specifically between the contracting parties, since each contract is usually different in some respect from the next.
