What form do I use to create an LLC in California?

In California, use Secretary of State Form LLC-1, "Articles of Organization." The form can be found in .pdf format along with instructions at the SoS Web site. I would recommend that any novice trying to form an LLC acquire and study one or more of the self-help paperback books on the subject of "how to set up and run your LLC in California" by publishers such as Nolo Press. In addition to the LLC-1, a newly-formed LLC will need an operating agreement between its members (and even a one-person LLC needs one) which serves the same general purpose as bylaws for a corporation.
