is there a form for gold mine partnership ownership?

There probably is some flaky form that has been used, in the past, to organize partnerships in the gold-mining industry. Frankly, I would NEVER use a "form" for any kind of partnership agreement, for any kind of business, without substantial personalization or other modification of its terms and conditions.

I don't have the foggiest idea what you and your prospective partners have in mind in terms of a gold mine partnership, but based on the usually speculative nature of mining partnerships and the high flexibility (and high personal risk) of any partnership, I would recommend obtainig legal advice and having a review of the risks you might be taking.
