licensing/release form

I am producing video biographies for individuals.

I would like them to sign a form which allows my company to show their final cut video in part or in whole for promotional purposes, as well as to be stored in a virtual library for future viewing by interested parties. (hopefully for a fee)

What type/kind of form/contract do I need?

Is there a source on the net where I can get it, or something I can modify, for free?

It is called a release. I don�t know where you can get free ones, try searching the net or asking to one from a similar business. They are not complex and it is not expensive for an attorney to draft one.

There are self-help law books that cover licensing and related topics (Nolo Press type). Probably not as widely distributed in bookstores as do your own divorce or form your own corporation. You can find them at Amazon or B&N's sites. I cannot vouch for their quality or usefulness but you may find a form that you can use or modify to fit your needs.
