If my LLC incurs a debt for services rendered, should a promissory note to repay be solely in the name of the said LLC, me individually or both?

It should be in the name of the LLC, and signed by someone authorized to enter into such a transaction on behalf of the LLC, with an annotation on the promissory note that the signer is signing on behalf of the LLC.
