There are about a dozen Secretary of State LLC forms, most simple and also accompanied by rather clear instructions. The management of any LLC should become familiar with them. The LLC-1 form is used to create the LLC; the LLC-2 is for changes in the organization and the LLC-10 for complete makeover. There is an LLC-12 Statement of Information form that needs to be filed biennially to keep the S/S current, and a couple of others used for dissolution. Go to www.ss.ca.gov and click the menus to get, eventually, to the LLC Forms.
