There are some forms that should accompany a patent application, but the process is not like getting a dog license or even as form-filing related as filing bankruptcy. A successful patent application is centered around, and totally dependent for its success upon, documents that the applicant creates from scratch. A novice patent applicant should go to the local Barnes & Noble or on-line at Amazon and get three or four cheap (compared with legal fees) self-help paperbacks on how to prepare and file a patent application. After carefully studying the patent-application process, the inventor can decide whether the idea is patentable at all; how to search for prior patents; and get a clearer understanding of what the USPTO requires from a patent applicant in the way of drawings, descriptions, claims, research into prior technology, and yes, "forms," in order to be a potentially successful applicant. Be assured, however, that obtaining a patent is no more a matter of finding and filling out the right form than getting happily married is a matter of filling out the right marriage license application.
