providing incorrect info to doctors

Hello,

The question is:

Is it legal for a patient to provide fake name and other details to a doctor when he is making all payments on the spot and in cash. In other words, does the patient has the right to presreve his privacy from the doctor himself in cases where all the fees and other charges are payed to the doctor upfront. The objective is to preserve the identity rather than avoiding payments.

What are the worst consequences of doing such a thing?

Depending on what "other details" you lie to your doctor about, the "worst consequence" is death.
