name remove off legal forms

how can the person who name is on the legal forms , have my name remove from that form i was the witnes and i put my name on it in front off them . i do not want to be a witnes how do iknow if they will take my name off like i have ask

i dont know what kind of form you are talking about so i cannot answer your question. however, if you witnessed ANYTHING and then refuse to testify in any future legal action you will go to jail.
